STATE OF ARIZONA INFORMATION
Hours: 8:00 a.m. to 5:00 p.m. State licensing is digital, all forms are available online and are submitted on the State Board Website at: https://massagetherapy.az.gov
For questions and to contact Board licensing staff email: info@massageboard.az.gov, the staff will respond by email.
The state now requires a NON-IVP fingerprint card to obtain or renew a massage therapy license. The information is on the state board website above and the form can be accessed at:
Select “Apply for a new card”
Select “Non-IVP”
Complete the printable pdf form, and send, or hand carry, the completed application, FD-258 fingerprint card and fee to DPS
Licenses now expire 24 months from the date of renewal, not on your birthday.
STATE BOARD OF MASSAGE THERAPY MEETINGS
